You can now complete your respirator medical evaluation questionnaire on-line and be certified by a doctor. Once you've begun working through the questionnaire, please finish. If you have any problems or questions, please call one of the numbers below during regular business hours PST.

This questionnaire will take approximately 5-10 minutes to complete.

All information that you provide is strictly confidential. To begin, it will be necessary to log on with the access code supplied by your Respiratory Protection Program Administrator.

Your employer must allow you to answer this questionnaire during normal working hours, or at a time and place that is convenient to you. To maintain medical confidentiality, your employer or supervisor must not look at or review your answers while you are completing the evaluation. To further protect the confidentiality of your responses, your medical information will not be stored in any capacity.
